If you have received a message from the PEN office about duplicates, you will need to complete the form below and attach it to a helpdesk ticket.  All fields are mandatory as we need to verify it against the existing record.  Admit the existing student to your school, and do not withdraw the duplicate.

When searching for a student, only enter the following:

1. Legal Last Name

2. Gender

3. Date of Birth

Any additional information will cause errors in the search and likely create a duplicate record.

We will assign the issue to L2 and they will delete the student in batches, so please be patient.
